What Does an NAD+ Supplement Do? Evidence and Forms

The direct answer: NAD+ (nicotinamide adenine dinucleotide) is an essential coenzyme involved in cellular energy metabolism, redox reactions and enzymes involved in processes such as DNA repair and cellular signalling. That biology is well established. What is much less settled is whether taking an NAD-related supplement produces meaningful healthy-ageing outcomes in people.
The most important distinction for shoppers is between direct NAD+ and the precursors NR (nicotinamide riboside) and NMN (nicotinamide mononucleotide). They belong to the same wider NAD conversation, but evidence from one should not automatically be used to prove the effect of another.
What does NAD+ do in the body?
NAD+ participates in oxidation-reduction reactions that help cells convert nutrients into usable energy. It is also consumed by enzymes including sirtuins, PARPs and CD38, which are involved in cellular signalling and maintenance pathways.
Those functions explain why NAD+ is prominent in ageing research. They do not mean that an oral supplement has been proven to increase energy, reverse ageing, extend lifespan or reproduce a cellular mechanism as a consumer outcome.
Does NAD+ always fall with age?
This common claim now needs more caution. A 2026 multi-cohort human study using a rigorously validated analytical method found that whole-blood NAD+ levels remained remarkably stable with age across seven independent cohorts. The authors concluded that whole-blood NAD+ may not be a useful biomarker of ageing or lifestyle factors.
That finding does not prove that NAD+ never changes with age in any tissue. Blood is not the same as muscle, brain, liver or other tissues, and different studies use different sampling and analytical methods. It does mean that statements such as “your NAD+ inevitably collapses as you age” are too broad.

A 2026 randomised human comparison found that 14 days of NR and NMN supplementation, but not nicotinamide, comparably increased circulating NAD+ concentrations in healthy adults. That result strengthens evidence that these precursors can engage the biochemical target. It is not a trial of direct oral NAD+.
What does the wider human evidence say?
A systematic review covering human and preclinical NAD-related intervention studies through October 2025 found that oral NR and NMN consistently demonstrated biochemical target engagement and were generally well tolerated over weeks to months. However, effects on functional, metabolic, vascular and other healthspan-related outcomes were heterogeneous and often null or specific to particular endpoints.
This is the key evidence boundary: raising a blood biomarker is not the same as proving a meaningful health or longevity outcome.
What about the brain and other tissues?
Newer work also shows why tissue context matters. A small 2026 pharmacokinetic study of oral NR and NMN reported that blood NAD increased over roughly two weeks and that cerebral NAD measurements increased after four weeks. The study involved only 12 participants and tested precursors, not direct NAD+.
It is therefore reasonable to say that NAD metabolism can respond to precursor supplementation in humans. It is not reasonable to convert those findings into a claim that any NAD supplement has been proven to improve cognition, energy or longevity.
Does “liposomal” prove better NAD+ absorption?
No. Liposomal describes a phospholipid-based delivery approach. A 2025 study comparing a liposomal NAD+ formulation with NAD+ alone reported differences in cultured human endothelial cells and keratinocytes, but this was an in-vitro cell study, not a human oral clinical trial.
That means liposomal formulation can be a legitimate product-design feature, but it should not be presented as proof of superior human bioavailability or better healthy-ageing outcomes without finished-product human data.
